In our latest installment of the Guidelines International Series, we will be looking at the latest guidelines from the International Society on Thrombosis and Haemostasis (ISTH). As the leading global organization in the field of bleeding and clotting disorders, ISTH plays a pivotal role in offering healthcare professionals and researchers worldwide with invaluable educational resources, networking opportunities, and essential professional tools.

The guidelines produced by ISTH are concise, targeted statements of expert opinion on the diagnosis and treatment of hemostasis and thrombosis disorders. These documents are authored by experienced clinicians who are esteemed authorities in the field and possess a deep understanding of the current medical and scientific literature. The primary aim of these statements is to offer guidance to healthcare providers in managing disorders for which there is a lack of data from randomized clinical trials to support evidence-based recommendations.

ISTH consistently releases a diverse array of international guidelines on an annual basis. Below, we present the guidelines from the past three years, along with their respective objectives.

2024 ISTH Guidelines

Treatment of Congenital Hemophilia A and B

  • Publication: June 19, 2024
  • This evidence-based clinical practice guideline is designed to offer a comprehensive overview of the evidence and to assist patients, caregivers, hematologists, pediatricians, and other clinicians, as well as researchers and stakeholders, in making informed treatment decisions regarding congenital hemophilia A and B.

Anticoagulation for Stroke Prevention in Atrial Fibrillation and Treatment of Venous Thromboembolism and Portal Vein Thrombosis in Cirrhosis

  • Previously, advanced liver disease was thought to only result in a bleeding disorder. However, there is now a growing understanding of a prothrombotic state that is associated with this condition. This puts patients at a higher risk for atrial fibrillation (AF), stroke, and venous thromboembolism (VTE), including portal vein thrombosis (PVT). In this review, we examine the existing literature on the epidemiology, pathophysiology, and risk factors related to this prothrombotic state in patients with cirrhosis. Additionally, we offer guidance on the management of anticoagulants for these conditions in adults with cirrhosis.

2023 ISTH update of the 2022 ISTH guidelines for antithrombotic treatment in COVID-19

  • ISTH released a general update to its guidelines for antithrombotic treatment in COVID-19, in response to the abundance of new randomized controlled trials (RCTs) published since the original guidelines were released in July 2022.

Management of Chemotherapy Induced Thrombocytopenia

  • Publication: October 10, 2023
  • This guidance document offers a thorough overview of the evidence and provides recommendations for the use of thrombopoietin receptor agonists (TPO-RAs) in different clinical settings, such as solid tumors, acute myeloid leukemia, stem cell transplant, and lymphoma.

Antithrombotic Treatment in COVID-19

  • Publication: July 05, 2022
  • (ISTH) has convened a diverse panel of international experts in content, patient advocacy, and methodology to formulate evidence-based recommendations regarding the use of anticoagulants and antiplatelet agents for patients with COVID-19 across various clinical scenarios. This panel has adopted the rigorous methodology established by the American College of Cardiology Foundation/American Heart Association to evaluate the level of evidence (LOE) and class of recommendation (COR) for these interventions.
